FDA RECORD / CURRENT STATUS
Can a pharmacy legally make TB-500 for you?
Not under the withdrawn request. Category 2 safety concerns remain, and the 2026 meeting produced no final FDA vote.
What the FDA review changed for pharmacy access
Can a U.S. pharmacy give you approved TB-500? No. FDA files name two forms called TB-500 free base and TB-500 acetate [18]. Free base is the plain chemical form; acetate has a salt added, but neither form gives you an approved medicine.
At the advisory meeting in 2026, FDA staff reviewed both forms beside BPC-157 and advised against adding them to the pharmacy list. The meeting ended without a recorded FDA vote, so as of September 2026 you can't treat that staff view as permission for your local drugstore to fill TB-500.
What FDA approval does not cover for TB-500
Is TB-500 approved for your medical care? No. The FDA hasn't found TB-500 safe and effective for any illness [17]. Federal records call TB-500 a thymosin beta-4 fragment. The request entered Category 2 on September 29, 2023, after regulators raised safety concerns.
That listing was a warning, not approval. It told your pharmacist that regulators saw risks and lacked enough safety proof. The record changed on April 22, 2026, when the applicant withdrew the TB-500 request [17]. Withdrawal stopped that request, but it didn't give you a safety clearance or approve thymosin beta-4 fragment TB-500. Your doctor can't prescribe it as an approved medicine.

What the former Category 2 warning means now
What did the old list mean for you? Section 503A is the federal law for local pharmacies making one person's custom medicine [16]. Category 1 held powders still being checked without an early safety flag [16]. Category 2 held powders with serious safety concerns [16].
The FDA used Category 2 when it feared bad immune reactions or lacked safety proof. Rules updated on January 7, 2025, barred your pharmacist from using those powders in a custom prescription [16][19]. You couldn't treat the old Category 2 listing as permission.
On April 22, 2026, FDA records said the applicant withdrew the TB-500 request [17]. That means the powder isn't an active Category 2 request now. Withdrawal didn't give you a safety clearance or drug approval. Your pharmacy still lacks a legal basis to use the powder.
What a lawful compounding pharmacy must have
How does lawful compounding work for you? A licensed doctor first examines you and decides that an approved drug won't meet your need [16]. The doctor then writes a prescription just for you [16]. A state-licensed pharmacy makes your drug under safety rules [16].
Federal rules don't let a pharmacy start with any powder it chooses. The powder needs a legal place on a government list or an official drug recipe [16]. The withdrawn TB-500 request doesn't supply that basis [17]. Your pharmacist can't make TB-500 from an unapproved powder with unresolved safety concerns [16].
Be wary if someone offers you easy access with no doctor visit [17]. A lawful clinic follows both state and federal rules. A ready-made vial sold online isn't the same as a drug made for your valid prescription. Without those checks, your health carries the risk.
What sports rules ban and FDA records still warn about
Can you use TB-500 in a tested sport? No. The World Anti-Doping Agency bans it both during and between events [11]. A lab can check your urine or blood, and horse racing has also caught the drug's use.
You may see posts saying an early-2026 change cleared TB-500 from Category 2 [17][18]. The FDA.gov record says less. The applicant withdrew the old Category 2 request, and FDA staff later advised against adding the powder to a pharmacy list. That former Category 2 file gives you no approval, because no final FDA vote is on record.
Is TB-500 legal?
Where and why matter. In the U.S., the FDA hasn't approved TB-500 for medical use [17]. The withdrawn Category 2 request gives pharmacies no basis to use the raw powder in a custom prescription [16]. Sports drug rules also ban it during and between events [11]. It's not an approved medicine.
Can you get TB-500 from a compounding pharmacy?
A normal compounding pharmacy has no legal basis to make TB-500 today [16]. The old Category 2 request from late 2023 was withdrawn, not approved [17][16]. A lawful pharmacy still needs a valid basis to use any raw powder. A vial offered without a doctor visit doesn't meet that test.
What is the FDA 503A status of TB-500?
Section 503A is the federal law for local compounding pharmacies. The FDA put the thymosin beta-4 fragment, sold as TB-500, in Category 2 [17]. That step took effect on September 29, 2023, after safety concerns. The applicant later withdrew its request; a review group then met in 2026 [18]. FDA staff advised against listing it, and no final vote is on record.
